BENEfit ® DIRECT Screws – The first angle-stable TAD BENEfit ® DIRECT S chrauben – Das erste winkelstabile TAD

Konventionelle TADs (Temporary Anchorage Devices) mit MA(R)PE (Miniscrew Assisted (Rapid) Palatal Expansion) Konventionelle Schrauben (TADs) eignen sich nicht opti- mal zur Fixierung von knochengetragenen palatinalen Ex- pandern, da sie keine stabile, formschlüssige Verbindung mit der Apparatur eingehen, sondern lediglich lose in der Verankerung liegen. Dadurch erfolgt bei der Expansion zunächst keine knöcherne Bewegung, sondern ein uner- wünschtes Kippen der Schrauben (Abb. 1). Zudem kann es aufgrund dieser Schräglage zu einer Verbiegung der Schrauben kommen. Im posterioren, häufig knochenär - meren Bereich erhöht sich das Risiko für Schraubenlo- ckerungen, Implantatverlust oder sogar Schraubenfraktu- ren. Infolge dieser Nachteile müssen mit herkömmlichen Schrauben fixierte Apparaturen meist über zusätzliche zahngetragene Elemente stabilisiert werden. Die Lösung: BENEfit ® DIRECT Schrauben für einen APPLIANCE FIRST Workflow. Die neuen BENEfit ® DIRECT Schrauben zeigen ihre Vor- teile insbesondere, wenn ein APPLIANCE FIRST Workflow bevorzugt wird. Ohne zusätzliche Software kann die Ap- paratur (z. B. Hybrid-Hyrax® , MA(R)PE mit unserem Labor- instrumentarium und den vorgeschweißten bmx® DIRECT Expandern direkt am Patientenmodell konstruiert werden. Die Implantatposition wird dabei präzise am Modell fest- gelegt. Die Apparatur kann anschließend an den Molaren fixiert werden, während die BENEfit ® DIRECT Schrauben sie sicher und stabil in der geplanten Position verankern. Das spezielle Gewindekopfdesign gewährleistet eine formschlüssige und kippsichere Verbindung zwischen DI- RECT-Platten und Implantat (Abb. 2) – ohne die Nachteile konventioneller Schrauben. Das neue smartlock ® BENEfit ® DIRECT winkelstabile TAD beseitigt all diese Probleme.

Conventional TADs (Temporary Anchorage Devices) with MA(R)PE (Miniscrew Assisted (Rapid)Palatal Expansion) Conventional screws (TADs) are not ideally suited for the fi - xation of bone-borne palatal expanders because they do not provide a stable, form-fit connection to the appliance, but instead remain loosely seated within the anchorage. As a result, the initial expansion does not lead to orthopedic ske- letal movement but rather to unwanted tipping of the screws (Fig. 1). Due to this unfavorable angulation, also screw ben- ding may occur. In the posterior region, where bone volu- me is often limited, this tipping effect increases the risk of screw loosening or screw loss. Consequently, appliances fixed with conventional screws typically require additional tooth-borne support for stabilization. The solution: BENEfit ® DIRECT screws for an APPLIANCE FIRST workflow. The new BENEfit ® DIRECT screws offer significant advantages when an APPLIANCE FIRST workflow is preferred. Without the need for additional software, the appliance (e.g., Hybrid- Hyrax ® , MA(R)PE can be prepared on the patient model using our laboratory instrumentation and pre-welded bmx ® DIRECT expanders. The exact implant position can be precisely de- termined and adapted directly on the working model. The ap- pliance can be secured to the molars, while the BENEfit® DI- RECT screws provide safe and stable fixation in the previously selected position. The special thread-head design ensures a reliable, rigid and tipping-resistant connection between the DIRECT plates and the implant (Fig. 2), avoiding the dis- advantages associated with conventional screws. The new smartlock ® BENEfit ® DIRECT angle-stable TAD elimi- nates all these problems.

Der Vorteil einer winkelstabilen Verbindung bei kieferortho- pädischen Behandlungen mit MA(R)PE MA(R)PE – M iniscrew A ssisted ( R apid) P alatal E xpansion (Minischrauben-gestützte Gaumennahterweiterung) Im orthopädischen/biomechanischen Kontext bezieht sich der Begriff „winkelstabil“ auf eine Implantatfixierungsme - thode, bei der die Schraube so in der Platte verriegelt wird, dass der Winkel zwischen Schraube und Platte fixiert und stabil ist. Es gibt kein „Wackeln“ oder Mikrobewegungen an dieser Schnittstelle. Da diese Schrauben-Platten-Verbindung starr ist, widersteht die Konstruktion Bewegungen (axial, Biegung, Rotation bes- ser. Studien zeigen, dass winkelstabile Konstruktionen eine höhere Primärstabilität (weniger Bewegung der Fragmente) im Vergleich zu herkömmlichen Schrauben/Systemen bie - ten. Bei Knochen minderer Qualität (posteriorer Gaumen- bereich) reduzieren winkelstabile Konstruktionen das Risiko einer sekundären Dislokation. Die smartlock ® BENEfit ® DIRECT TADs verfügen über zwei verschiedene Gewinde im Schraubenkopf und im Knochen- gewinde, wodurch verhindert wird, dass das System auf das Zahnfleisch drückt und die Blutversorgung weniger beein - trächtigt wird. Selbst wenn die TADs nicht in einem 90°-Win- kel zur Platte platziert werden, bleibt die Stabilität erhalten, da eine Abweichung von bis zu 15° toleriert wird. Vorteile: ▪ Es können kürzere Schrauben verwendet werden. 11 mm ist eine gängige Größe ▪ Keine unerwünschte Bewegung an der Verbindungsstelle Platte/Schraube ▪ Apparatur wird leicht nach oben gehoben und drückt nicht auf die Schleimhaut ▪ Es wird keine zusätzliche dentale Abstützung benötigt ausser bei Hybrid Hyrax ® ▪ Keine Verbindung Platte/Schraube mit Komposit oder ähnlichem Material notwendig

The advantage of an angle-stable connection in Orthodontic treatments with MA(R)PE MA(R)PE – M iniscrew A ssisted ( R apid) P alatal E xpansion In an orthopaedic/biomechanical context, the term “angle- stable” refers to an implant fixation method in which the screw is locked into the plate in such a way that the angle between the screw and the plate is fixed and stable. There is no “wobbling” or micro motion at the interface. As this screw-plate interface is rigid, the construct resists movement (axial, bending, rotational) better. Studies show that angle-stable constructs provide higher primary stability (less movement at fragments) compared with conventional screws/devices. In bones with poor quality (posterior region of the palate) angle-stable constructs reduce risk of seconda- ry displacement. The smartlock ® BENEfit ® DIRECT TADs feature two different threads in the screwhead and the bone thread which pre- vents the device to be pressed to the gingiva causing less disturbance of the blood supply. Even if the TADs are not placed in a 90° angle to the plate the stability is maintained as it tolerates a deviation of up to 15°. Benefits: ▪ Shorter screws can be used. 11 mm is a common size. ▪ No unwanted movement at the plate/screw connection point. ▪ The appliance is slightly elevated and does not press on the mucous membrane. ▪ No additional dental support is needed except with Hybrid Hyrax ® . ▪ No bonding of the plate/screw connection with composite or similar material is necessary.

Sterile Packaging | Sterile Verpackungen

Implants – in single sterile packaging The outer and inner packaging contains labels indicating UDI code – like article number, LOT number, expiration date, etc. for the patient folder and for full traceability. The sterile screws are contained in a small colour-coded screw carrier which protects the screw and at the same time acts as a pick up tray. This carrier is placed in a clear, sealed blister package. The screw can easily be removed from the carrier by means of the self retaining screwdriver. Implantate – in steriler Einzelverpackung Die äußeren und inneren Verpackungen enthalten genügend Klebeetiketten mit UDI Angaben der Artikelnummer, LOT Nummer, Haltbar- keitsdatum, etc. für die Patientenakte und für die genaue Rückverfolgbarkeit. Die sterilen Schrauben werden in einem kleinen, farbcodierten Schraubenträger aufbewahrt, der die Schraube schützt und gleichzeitig als Entnahmebehälter dient. Dieser wiederum befindet sich in einem durchsichtigen, versiegelten Blister. Die Schraube kann ohne zusätz - liche Berührung, wie gewohnt mit dem Schraubendreher aus dem Schraubenträger entnommen werden. Es ist keinerlei Aufbewahrungs- oder Entnahmebehälter mehr notwendig.
